- Need help with transport
- Need some in-home care and companionship support (if 65 or over)
- Are at risk of losing your independence

For those aged 65 and over
If you are aged 65 or over (or 50 years or over if you are Aboriginal and Torres Strait Islander) you may qualify for subsidised services under the Commonwealth Home Support Programme (CHSP). The CHSP helps you stay independent and in your home and community for longer.
Services provided under the CHSP include:
- Social support - social activities in a community-based group setting
- Transport - to take you to medical appointments, shopping or for social purposes
STAR can provide these services.
To find out if you qualify for subsidised services under CHSP, please call on My Aged Care Phone Number 1800 200 422.
About My Aged Care
The My Aged Care website has been established by the Australian Government to help you navigate the aged care system. My Aged Care is part of the Australian Government’s changes to the aged care system which have been designed to give people more choice, more control and easier access to a full range of aged care services.
If MyAgedCare consider you eligible for subsidised transport or social support, they will in most cases arrange to visit you for a Home Support Assessment. If you are approved for transport and/or social support, MyAgedCare will send us a referral. Our intake team will then contact you to determine how we can assist you.
Please note, if you specifically wish STAR to be your transport provider, you need to inform MyAgedCare.
For those aged under 65
If you are under 65 years of age (or under 50 years if you are Aboriginal and Torres Strait Islander) you may qualify for subsidised transport services under the Queensland Community Care Services program. To find out if you are eligible, please contact Community Access Point on 1800 600 300.
If you meet the criteria for subsidised transport Community Access Point will arrange to visit you at home for a full assessment. If they approve you for transport services they will issue us with a referral. Our intake team will then contact you to determine how we can assist you.
