Help Older Australians Live Safely and Independently at Home

In this vital role, you will coordinate and monitor aged care services to ensure they are person-centred, wellness-focused, and compliant with the Commonwealth Home Support Program and the Strengthened Aged Care Quality Standards. You’ll work directly with Participants, contractors, and allied health professionals to make homes safer and lives more connected.

You’ll also:

  • Develop and review service plans aligned with client goals
  • Coordinate internal and external service providers (e.g., tradespeople, support workers)
  • Manage schedules, work orders, and rostering for services
  • Ensure accurate documentation and compliance reporting (including DEX)
  • Maintain positive relationships with Participants, carers, contractors, and stakeholders
  • Contribute to quality improvement, audits, and team-based service planning

What will you bring to the role?

·                2+ years’ experience in aged care, ideally in CHSP service coordination

·                Understanding of the Aged Care Act 2024 and CHSP program requirements

·                Knowledge of the incoming support at home program and strengthened standards is highly desirable

·                Knowledge of wellness and reablement principles and quality standards

·                Strong customer service and communication skills

·                Ability to work with diverse teams and stakeholders

·                Excellent organisational, rostering, and record-keeping skills
