In this vital role, you will coordinate and monitor aged care services to ensure they are person-centred, wellness-focused, and compliant with the Commonwealth Home Support Program and the Strengthened Aged Care Quality Standards. You’ll work directly with Participants, contractors, and allied health professionals to make homes safer and lives more connected.

You’ll also:‍

• Coordinate CHSP Group & Individual Social Supportservices for older people living in the community
• Design, implement, and review tailored activity programsthat support social engagement and quality of life
• Conduct client assessments, goal setting, care planning, andregular reviews in line with CHSP requirements
• Coordinate service delivery and provide support and guidanceto frontline staff
• Work with a diverse client group, including clients withcomplex needs such as hoarding and squalor
• Liaise with families, carers, internal teams, and externalstakeholders
• Ensure accurate documentation, reporting, and data entry tomeet CHSP compliance standards
• Travel as required to Ipswich and Lowood, with therole based in Cleveland
