STAR Community Services is seeking a dynamic Client Planning and Service Delivery Manager to lead a multidisciplinary team and champion high-quality, person-centred care under the Support at Home Program and NDIS.
As the Client Planning and Service Delivery Manager, you will provide strategic and operational leadership across multidisciplinary teams, delivering aged care and NDIS services. The role ensures high-quality, person-centred, and compliant service delivery aligned with the Aged Care Act 2024, Strengthened Quality Standards, and NDIS Practice Standards.
Key Responsibilities
Clinical and Operational Leadership
· Provide clinical guidance aligned with aged care and NDIS standards
· Lead complex case management, policy adherence, and quality practice
Team Leadership & Supervision
· Manage, mentor and support multidisciplinary teams
· Foster a respectful, inclusive culture focused on safety and performance
Person-Centred Planning
· Oversee development and review of care/support plans aligned with goals and compliance
· Support collaborative planning with clients, families, and other providers
Service Oversight
· Manage home and NDIS service delivery to meet assessed needs and KPIs
Resolve complaints and oversee service utilisation
Quality & Compliance
· Ensure adherence to relevant legislation and standards
· Conduct internal audits and lead continuous improvement
Risk and Incident Management
· Identify, document, and manage clinical risks and incidents
· Lead investigations and ensure compliance with reporting protocols
Stakeholder Engagement
· Liaise with external stakeholders and represent STAR in forums
· Maintain strong, respectful client relationships
Innovation and Evidence-Based Practice
· Champion wellness and reablement models
· Lead integration of evidence-based practices
Workforce Development
· Identify training needs and provide professional development
· Promote continuous learning and supervision
Data, Reporting, and Financial Management
· Monitor KPIs, clinical outcomes, and budgets
· Collaborate with finance to ensure proper claims and allocations
Staff Wellbeing and Culture
· Promote safe work practices and psychological safety
· Support WHS, staff wellbeing, and a positive team culture
What will you bring to the role?
- Bachelor in Nursing/Allied Health; AHPRA registration
- 3+ years in clinical/care management (home/community-based care)
- Knowledge of aged care reforms and NDIS
- Budget and performance monitoring experience
- Strong leadership, compliance, and IT skills
