Age Care Coordinator Social Support - Commonwealth Home Support Program

We are seeking a skilled and compassionate Coordinator – Aged Care Social Support (Group & Individual) to coordinate and oversee CHSP-funded social support services for older people living in the community.

The primary focus of this role is coordinating person-centred, activity-based group & individual programs that promote independence, wellbeing, and social connection for older people. The role aligns closely with a Diversional Therapist or similar background and will suit someone experienced in developing meaningful programs for aged clients.

In addition to general CHSP social support clients, the role will also work with clients experiencing hoarding and squalor, requiring a sensitive, trauma-informed, and non-judgemental approach.
