As the Clinical Care Partner RN, you will play a vital role in supporting participants with diverse and complex health needs by providing advanced clinical nursing assessment, care coordination, clinical consultancy, nursing interventions, care planning, delegation and clinical leadership. You will act as a key clinical link between participants, their families and the health and community services that support them.
In this role, you will combine advanced clinical nursing expertise, care coordination, delegation, clinical leadership and relationship-based practice to support participants in achieving their goals while promoting wellness, independence and re-ablement. Acting as a clinical resource and subject matter expert, you will support evidence-based practice, contribute to clinical governance, lead clinical decision-making and assist in building workforce capability across the Support at Home program.
You will provide clinical leadership, supervision and oversight to Care Partners and nursing staff, ensuring delegated clinical activities are delivered safely, effectively and in accordance with professional nursing standards, organizational policies and the Strengthened Aged Care Quality Standards.
