STAR Community Services is seeking an experienced and compassionate Registered Nurse to join our Client Planning & Service Delivery team as a Clinical Care Partner on a fixed-term contract. This is a rewarding community-based role where you will provide high-quality clinical nursing care to older people with complex health needs, supporting independence, reablement and person-centred outcomes within the Support at Home Program.
About the Role
As a Clinical Care Partner – RN, you will provide clinical leadership, assessment and direct nursing care while working closely with Care Partners and a multidisciplinary aged care team. You will play a key role in developing, implementing and monitoring clinical care plans and ensuring nursing care is delivered safely, effectively and in line with Strengthened Aged Care Quality Standards.
